I voluntarily assume all risks and agree to release and hold harmless Formula M Creatives, Inc. DBA Mic Diaz Presents, its team, partners, sponsors, and venue from any liability arising from my attendance.

I agree to follow all event and venue rules and understand I may be removed for non-compliance.

I also grant permission to Formula M Creatives, Inc. DBA Mic Diaz Presents to capture and use my image, likeness, and voice in photo/video content for promotional and media purposes without compensation.

I confirm that I am responsible for my own actions and conduct during the event.
